What is it?
The difference between Capital D in Deaf and lowercase d in deaf is that Deaf is associated with culture meanwhile deaf is associated with being medical.

People who are part of the deaf community normally don’t associate with others who are deaf. They aren’t interested in learning more about Deaf culture and barely know how to sign. They either have hearing loss or can’t hear at all. They also have not attended a school for sign language or deaf people.
People who are a part of the Deaf community are very educated in many aspects of the Deaf community. They are fluent in sign language and have or still attend a deaf school. They are a strong member of the Deaf community.
